Advanced Innovation Programme on Chronic Conditions Management - programme outline

The programme’s principal objective is to provide a workable model for the holistic management of chronic conditions for EU citizens, taking into account the medical, socio-economic and technology dimensions. The model will be based on a benchmark of best practices across Europe with the aim of establishing a European standard that can be used by EU Member States in their non-disease specific national plans for the management of chronic conditions. It will focus on the commonalities across conditions but use some specific disease areas as case studies to illustrate particular aspects of the model.
