A one-day conference open to all stakeholders.
Following the presentation of the draft Optimal Chronic Care Model in November 2011, this follow-up conference will present and discuss the results of the 2012 work programme:
• Pilot tests of the model "in situ" across various stakeholder groups, with a particular focus on community (informal care-givers), employment and work-life interactions and settings
• Application of the model to rare disease (one of the research control groups)
• Implementation frameworks arising from the pilot tests
• Establishing benchmarks and monitoring processes.
