A one-day multi-stakeholder conference to:

  • present and discuss Epposi's full research findings
  • test the outline model using Decide, an EU-funded experiential learning approach designed to help organisations, individuals and public administrations find consensus and concrete outcomes to interlinked and multilevel challenges.
  • set the scope for an implementation framework for a patient-centred approach to chronic conditions management
  • report on Epposi's work on 'care and cure' with the pilot European Innovation Partnership in Active and Healthy Ageing (EIP-AHA)
